The only way that could happen is if his foot was dangling outside the counterbalance. Not going to dox my business but it involves this shit. Majority of forklift accidents happens to drivers with 5+ years experience, because they get complacent af (sassy).

Some young kid on a site I was working on took a heavy ass pallet down a ramp backwards. He lost control of the pallet jack and it smashed his foot against the concrete wall. Pulped that shit. I felt bad for him because sure it was stupid but that's what foremen are there for - keep the young guys from killing themselves.

When I went for my forklift licence I was like "wtf they have seatbelts?" I never wore one, but the only reason I would is if I felt I was in an unsafe situation, like getting hit with a truck... in which case I wouldn't be in a forklift.
