Twice I've gotten the gold star in Final Fantasy 8 by collecting every single card. I got all the GFs too. I also got the certificate by beating Omega Weapon (hit the nigga with Meltdown and Renzokuken over and over).

My dorm had Galaga in the lobby, I got the high score on my first weekend there and remained on top the entire year.
I have no proof of this, because back then any sort of documentation of one's video game achievements would have been considered super, super gay, and would have rightfully gotten you ridiculed or beaten.
