I think its as simple as fat lazy white bitch goes to the doctor complaining of pain. This “pain” is either caused by being a fat fuck, being a lazy fuck, or they’re on their period.

The doctor can no longer say “Maybe you need to work out / are imagining this, you fat bitch, come see me every month so we can track your weight loss, then tell me if the pain subsides. If it doesn’t we might be dealing with a real issue.”
